Fasting in Ramadan is an obligation (fardu) upon every Muslim who is of age and able (Surah Al-Baqarah 2:183). Yet Allah SWT is Most Merciful — He grants a rukhsah (concession) to those with a valid excuse, including the sick. Rukhsah means a relaxation of a ruling that Allah permits because of a hardship; it is not a breach of the Shariah, but part of the Shariah itself.

فَمَن كَانَ مِنكُم مَّرِيضًا أَوْ عَلَىٰ سَفَرٍ فَعِدَّةٌ مِّنْ أَيَّامٍ أُخَرَ

"Maka sesiapa di antara kamu yang sakit atau dalam musafir (lalu dia berbuka), maka (wajiblah dia berpuasa) sebanyak hari yang ditinggalkan itu pada hari-hari yang lain.""So whoever among you is ill or on a journey (and breaks the fast) — then an equal number of other days [are to be made up]."

Pesakit yang berbuka atas nasihat perubatan yang sah hendaklah menggantikan puasa (qada) apabila sihat kemudian.Taking a concession while sick is not a sign of weak faith. It is obedience to the Allah who granted that ease — just as He loves His servant to perform the obligatory, He also loves His servant to accept the concessions He permits. 2. Apa Yang Membatalkan Puasa (Konsep Rongga / Al-Jawf)2. What Invalidates the Fast (The Concept of the Cavity / Al-Jawf)

Secara ringkas, dua perkara utama yang membatalkan puasa ialah makan dan minum dengan sengaja, serta memasukkan sesuatu ke dalam rongga terbuka badan (dikenali dalam fiqh sebagai al-jawf). Ramai pesakit bimbang: "Jika saya terpaksa berbuka kerana rawatan, adakah saya kehilangan ganjaran Ramadan?" Fahami dahulu keutamaan berpuasa Ramadan dengan penuh keimanan dan mengharapkan pahala: Many patients worry: "If I have to break my fast for treatment, do I lose the reward of Ramadan?" First, understand the virtue of fasting Ramadan with full faith while hoping for its reward:

Dalam satu riwayat lain, Rasulullah ﷺ menerangkan bahawa apabila seseorang hamba jatuh sakit atau bermusafir, Allah tetap mencatatkan untuknya pahala amalan yang biasa dia lakukan ketika sihat dan bermukim (riwayat Sahih al-Bukhari, daripada Abu Musa al-Ash'ari R.A.). Maknanya, seorang Mukmin yang terhalang beribadah kerana keuzuran tidak dizalimi pahalanya — Allah Maha Adil dan Maha Pemurah. For a patient who is genuinely unable to fast, Islam offers reassuring glad tidings. In another narration, the Prophet ﷺ explained that when a servant falls ill or travels, Allah still records for him the reward of the deeds he habitually performed when healthy and resident (narrated in Sahih al-Bukhari, from Abu Musa al-Ash'ari R.A.). This means a believer prevented from worship by a valid excuse is not deprived of the reward — Allah is Most Just and Most Generous.

Justeru, mengambil rukhsah dan berbuka atas nasihat perubatan yang sah, kemudian menggantikan puasa (qada) apabila sihat, ialah satu bentuk ketaatan — bukan kekurangan iman. Pesakit tidak perlu merasa bersalah; dia sedang mengikut jalan yang Allah izinkan. Therefore, taking the concession and breaking the fast on valid medical advice, then making it up (qada) once healthy, is a form of obedience — not a lack of faith. A patient who breaks the fast for a valid Shariah reason must make up (qada) the number of days missed on other days once healthy, as Allah states in Surah Al-Baqarah (2):185. If the illness is prolonged or permanent such that fasting is not possible at all, refer to authoritative scholarly opinion (a fatwa body/mufti) regarding the payment of fidyah.
