Sokongan Spiritual Untuk Pesakit: Empati, Bukan Simpati. Keberadaan, Bukan Kesempurnaan.Spiritual Support for Patients: Empathy, Not Sympathy. Presence, Not Perfection.
Intinya mudah: empati, bukan simpati; keberadaan, bukan kesempurnaan. Many of us know what to bring when visiting someone unwell: a small gift, medicine, a card. Yet once we sit at the bedside, we suddenly find ourselves lost for words. What do you say to someone who is ill? What words of encouragement truly help a patient, and how do you comfort them without adding to their burden? This article is a spiritual support guide drawn from the Medical Fiqh Workshop by the Shariah Compliance Department of An-Nur Specialist Hospital, written for families, visitors and healthcare workers. Its heart is simple: empathy, not sympathy; presence, not perfection.
1. Memahami Di Mana Posisi Kita1. Understanding Where We Stand
Lima posisi yang dikenal pasti: Before thinking about the right words, the workshop invites us to pause and ask: who are we to the person pouring out their feelings? In the workshop, that person is called the confider. Where we stand in relation to the confider determines the role we should play. Five positions are identified:
1. Si peluah sendiri1. The confider themselves
The person going through the trial and expressing what they feel.
2. Kawan rapat2. A close friend
Closest to the confider and usually the first person they turn to.
3. Kenalan3. An acquaintance
Most acquaintances do not know the confider's true circumstances.
4. Figura autoriti4. A figure of authority
Teachers, counsellors, psychologists and doctors, approached for their knowledge and expertise.
5. Pemimpin masyarakat5. A community leader
Not in direct contact with the confider, yet their decisions shape the economic and welfare policies that affect the confider's life.
Different positions carry different roles. The common mistake is that we speak beyond our position: an acquaintance talking as though they were an expert, or a visitor trying to take over the doctor's role. Knowing our own position is the first step towards offering a patient emotional support with proper manners.
2. Peranan Pertama: Empati, Bukan Simpati2. First Role: Empathy, Not Sympathy
The first role the workshop stresses is empathy, not sympathy. Empathy means imagining ourselves in the confider's place, then trying to fathom what they are going through and how they feel in that moment. It is different from simply feeling sorry from a distance.
With empathy, we should not be quick to hand out advice, offer solutions, or pass judgement based on our own perspective alone. Instead, empathy should help us recognise and respect the unique context of the confider's life. Trials that look similar are never equally heavy for two different souls.
Perhatikan perbezaan dua ayat ini: One important reminder: do not be quick to assume we understand the confider merely because we have been in the same or a similar situation. This is where attempts to comfort a sick person often slip, even with the best of intentions. Notice the difference between these two sentences:
ElakkanAvoid
"Saya faham awak, saya pun pernah laluinya." I understand you, I have been through it too." This sentence shifts the focus to our own story, and assumes their experience mirrors ours.
Cuba IniTry This
I honestly cannot imagine you having to face all this. It must be so challenging, right?" This sentence honours the uniqueness of their trial and opens space for them to keep sharing.
3. Spektrum Empati: Daripada Belas kepada Kasih Sayang3. The Empathy Spectrum: From Pity to Compassion
The workshop pictures empathy as a spectrum, not an on-off switch. Its axis is effort and understanding: the greater our effort to understand, the deeper the connection that forms. This is also the simplest answer to the often-asked question of empathy versus sympathy.
Empat Tahap Merasai BersamaFour Levels of Feeling Together
Paksi: usaha memahami lawan kedalaman hubunganAxis: effort to understand versus depth of connection
BelasPity
"Saya kasihan pada awak.""I feel sorry for you."
SimpatiSympathy
"Saya rasa dengan awak.""I feel with you."
EmpatiEmpathy
"Saya rasa untuk awak.""I feel for you."
Kasih SayangCompassion
"Saya nampak dan faham awak.""I see and understand you."
Notice that sympathy, "I feel with you", is not wrong in itself. It simply has not yet reached the level of empathy, "I feel for you", a more deliberate effort to enter the confider's world. The peak is compassion, when the confider feels truly seen and understood.
4. Peranan Kedua: Keberadaan, Bukan Kesempurnaan4. Second Role: Presence, Not Perfection
Peranan kedua ialah keberadaan, bukan kesempurnaan. Matlamat kita bukan menyediakan jawapan yang sempurna, tetapi membuatkan si peluah merasakan kita benar-benar bersamanya: dia diberi sepenuh perhatian, dan dia bukan beban untuk kita. Tiga amalan mudah membantu: The second role is presence, not perfection. Our goal is not to supply the perfect answer, but to make the confider feel we are truly with them: that they have our full attention, and that they are not a burden to us. Three simple practices help:
- Beritahu dengan jelas bahawa kita bersedia mendengar. Kadangkala pesakit menunggu isyarat itu sebelum berani membuka cerita.Say clearly that we are ready to listen. Sometimes a patient waits for that signal before daring to open up.
- Jaga bahasa badan. Duduk berdekatan, pandang wajahnya, dan biarkan tubuh kita menyatakan bahawa dia selamat untuk bercerita.Mind our body language. Sit close, face them, and let our posture tell them it is safe to talk.
- Hadir dahulu. Kadangkala fokus kita teralih kerana sibuk memikirkan solusi dan respons ketika dia sedang meluah. Ketepikan dahulu semua itu; dengar sahaja.Be present first. Our focus sometimes drifts because we are busy composing solutions and responses while they are still pouring their heart out. Set all that aside for now; just listen.
Respect and acknowledge their feelings, even when those feelings are heavy to hear. This is where we need to be careful with toxic positivity. Phrases like "Be patient, there is wisdom in this" or "Just stay positive, do not be sad" are in fact good words, and there are moments when they are truly needed. But when spoken too early, before we have genuinely listened, they can make the confider feel their emotions have been dismissed. Words of encouragement mean the most when they come after we listen, not in place of listening.
5. Teladan Sirah: Baginda SAW dan Khadijah R.Anha5. A Model from the Sirah: The Prophet SAW and Khadijah R.Anha
The finest model of both roles comes from the sirah of the Prophet Muhammad SAW himself. After receiving the first revelation in the Cave of Hira', he returned to his wife Khadijah R.Anha gripped by fear, and asked: "Cover me with a blanket."
Beliau menyelimuti Baginda sehingga reda ketakutannya, menenangkan Baginda, lalu mengiktiraf segala kebaikan Baginda dengan kata-kata yang masyhur: Notice what Khadijah R.Anha did not do. She did not judge, did not correct, and did not fire off question after question. She wrapped him in a blanket until his fear eased, comforted him, then affirmed all the goodness in him with her famous words:
By Allah, Allah will never disgrace you..."
Sebagaimana diriwayatkan dalam hadis permulaan wahyu (Sahih al-Bukhari)As narrated in the hadith of the beginning of revelation (Sahih al-Bukhari)
That is presence and empathy in their purest form: covering before counselling, calming before questioning, and affirming a person's goodness at the very moment fear engulfs him. If we are looking for guidance on what to say to someone who is ill, start here: soothe first, affirm their goodness, and let them know they are not alone.
6. Pada Akhirnya: Hadir Tanpa Putus Asa6. In the End: Present Without Giving Up
Finally, the workshop puts our goal back in its rightful place. We become good listeners not with the primary objective of seeing the person quickly heal and recover. We want to show empathy and presence continuously, without giving up, however long the trial drags on.
Our presence beside someone, even without words, can make that person feel they are not alone. That is the heart of it: presence matters more than the correct and perfect response. For those who wish to complete their presence with practice, the healing supplications of the Prophet SAW are gathered in our article on prayers for patients.
And for our fellow healthcare workers, this gentle reminder is for all of us. When other people's pain becomes daily routine, it is easy to grow numb. Bring empathy to the patient even when pain has become familiar to us; this is part of the culture of care at An-Nur Specialist Hospital.

Apakah beza empati dan simpati? What is the difference between empathy and sympathy?
Sympathy is feeling alongside someone, for example "I feel with you." Empathy goes further: we imagine ourselves in their place and try to fathom what they are going through and how they feel in that moment. With empathy, we do not casually hand out advice or judge from our own perspective alone; instead we recognise and respect the unique context of their life.
Macam mana kalau saya tak tahu nak cakap apa? What if I do not know what to say?
