As the Third Trimester approaches, the joy of becoming a mother begins to mix with anxiety. Among the most common questions running through a mother's mind (especially first-timers) are: "Will pushing hurt a lot? What if I tear badly? I'm so scared if the doctor has to 'snip' down there!"

This snipping procedure, known as an Episiotomy, is often a nightmare for many. However, did you know that vaginal muscles are naturally designed to stretch? The key to reducing the risk of severe tearing is to push using the correct technique.

5 Secrets to Pushing in the Labor Room

  • Jangan Angkat Punggung: Kesilapan paling besar! Bila anda angkat punggung dari katil semasa meneran, tekanan yang salah akan terarah pada perineum, menyebabkan ia sangat mudah koyak. Rapatkan punggung ke tilam.
  • Teran Di Perut, Bukan Di Leher: Bayangkan anda sedang membuang air besar yang sangat keras (sembelit). Jangan kumpul angin di muka (muka jadi merah padam) atau menjerit. Salurkan tenaga ke bahagian bawah perut dan pelvis.
  • Dengar Arahan Doktor / Bidan: Ada masanya doktor akan suruh anda BERHENTI meneran dan hanya 'pant' (bernafas pendek-pendek). Ini sangat penting untuk memberi masa otot meregang perlahan-lahan supaya tidak terkoyak.
  • Tarik Nafas Panjang & Tahan: Bila kontraksi datang memuncak, tarik nafas dalam-dalam, rapatkan dagu ke dada, dan teran sekurang-kurangnya selama 10 saat tanpa melepaskan nafas.
  • Amalkan Urutan Perineum: Mulakan urutan minyak zaitun/VCO di bahagian perineum sejak minggu ke-34 kehamilan untuk melembutkan dan menganjalkan otot di kawasan tersebut.
  • Do Not Lift Your Buttocks: The biggest mistake! Lifting your buttocks off the bed while pushing directs the wrong pressure onto the perineum, making it highly susceptible to tearing. Keep your bottom flat on the mattress.
  • Push from the Abdomen, Not the Neck: Imagine you are passing a very hard stool (constipation). Don't hold the air in your face (face turning red) or scream. Channel the energy downwards to your abdomen and pelvis.
  • Listen to the Doctor / Midwife: There will be moments when the doctor tells you to STOP pushing and just 'pant' (take short breaths). This is crucial to give the muscles time to stretch slowly and prevent tearing.
  • Take a Deep Breath & Hold: When the contraction peaks, take a deep breath, tuck your chin to your chest, and push for at least 10 seconds without letting the air out.
  • Practice Perineal Massage: Start massaging the perineum with olive oil/VCO from week 34 of pregnancy to soften and elasticize the muscles in that area.
